By executing this application, the undersigned person certifies to the following: The information set forth in this application and in the documents, schedules, reports, statements, and any other information provided to Apex CFS (“Apex CFS”) with or pursuant to this application are full, true, correct, complete, and accurately reflect such information on the date thereof; that Apex CFS is authorized to request, receive, and verify credit reports and other financial information regarding applicant and its business that Apex CFS deems necessary and appropriate; that Apex CFS is authorized to execute in the name of the undersigned person and file against the undersigned person in favor of Apex CFS, UCC’s and financing statements with respect to the undersigned person’s assets; and that Apex CFS is authorized to inquire of, investigate, confirm, and verify any information contained in the application, in any documents, schedules, reports, statements, and any other information provided under or pursuant to this application, or learned by Apex CFS as part of its investigation and review of this application, applicant, or applicant business. Applicant agrees to reimburse Apex CFS for any and all costs incurred by Apex CFS as a result of applicant’s withdrawal of this application prior to the first funding. What this means for you: when you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents for your business.
